Overview

Coronary artery bypass grafting remains the bread and butter of most cardiac surgeons in the UK. Around 85% of cases are still performed with an arterial conduit and vein grafts. While this remains the mainstay of practice in the UK,  large multicenter studies have highlighted the benefits of multiple arterial grafts. While we await the final results of the ROMA trial (Expected in 2026), we present an opportunity to gain practical knowledge from seasoned surgeons on performing total arterial CABG. This session covers conduit selection, stabilisation strategies, and common pitfalls—empowering you to refine your technique and improve patient outcomes.

Aims

  • Educate fellow surgeons and trainees on advanced techniques in coronary artery bypass grafting using total arterial conduits.
  • Share insights highlighting outcomes, patient selection, and navigation of difficult scenarios.
  • Demonstrate practical approaches to Pure Internal Thoracic Artery Revascularisation and Off-Pump Total Arterial Revascularisation, including conduit choice, stabilisation methods, and avoidance of common pitfalls.
  • Encourage discussion and exchange of ideas among peers to advance the field of durable coronary surgery.

By the end of this webinar, attendees should be able to:

  • Understand the principles and benefits of Total Arterial revascularisation,  including its impact on long-term graft patency
  • Identify appropriate patient selection criteria for total arterial revascularisation strategies, with a focus on bilateral ITA use.
  • Describe key technical steps in performing Off-Pump Total Arterial Revascularisation, including conduit harvesting, anastomosis techniques, and stabilisation methods.
  • Recognise common pitfalls and complications associated with off-pump and total arterial CABG, and apply strategies to avoid or manage them.
  • Apply practical tips and tricks shared by expert surgeons to enhance procedural efficiency and patient safety in total arterial CABG.

Professor Nawwar Al-Attar - Consultant Cardiac Surgeon, Golden Jubilee National Hospital, Honorary Clinical Associate Professor, University of Glasgow.

Professor Nawwar Al-Attar is a Consultant Cardiac Surgeon at the Golden Jubilee National Hospital in Glasgow, Scotland. He is also an Honorary Clinical Associate Professor at the University of Glasgow and is a Specialty Advisor for Cardiac Surgery to  Chief Medical Officer in Scotland. He has many research and academic contributions including the TAVI-UK VICTORY CT001 Study and FAME 3. He also serves as part of the European Society of Cardiology (ESC) Working Group on Cardiovascular Surgery. His primary interests include Pure Internal Thoracic Artery (PITA) Coronary Artery Bypass Grafting.

 

Professor Vipin Zamvar - Consultant Cardiothoracic Surgeon, Royal Infirmary of Edinburgh, Editor-in-Chief of the Journal of Cardiothoracic Surgery.

Mr. Vipin Zamvar is a Consultant Cardiothoracic Surgeon at the Royal Infirmary of Edinburgh, with expertise in advanced cardiac and thoracic procedures. Born in Mumbai, India, he has built a remarkable career in the UK, combining clinical excellence, research leadership, and a passion for teaching. He has been lead investigator in several multi-centre trials on conduits and techniques in CABG surgery. Academically, he is the current Editor In Chief of the Journal of Cardiothoracic Surgery and has published extensively on OPCAB outcomes, patient safety, and long-term graft patency. He was the president of the World Society of Cardiovascular and Thoracic Surgeons in 2015 and is currently the general secretary. Beyond medicine, he is also a dedicated chess enthusiast, drawing parallels between strategy in chess and precision in surgery.
